2023-2024 East Hartford Community Health Needs Assessment
Local and state health departments have requirements and obligations to ensure the health and well-being of those who live, work, and visit their communities. The East Hartford Department of Health and Human Services (EH-DHHS) is committed to enhancing the quality of life for residents of East Hartford and supporting the overall health of the community through disease investigation, injury prevention, environmental protection, public health preparedness, and health promotion.
The primary purpose of the Community Health Needs Assessment (CHNA) was to identify community health priorities in order to implement strategies to improve health behaviors. The EH-DHHS conducted the CHNA in 2023-2024 to meet the following goals:
- Understand the demographic makeup of East Hartford to center efforts in health equity and address barriers in ways that are accessible and culturally competent.
- Identify the health behaviors and health concerns that are impacting East Hartford residents.
- Prioritize the health needs of specific groups within East Hartford.
- Develop a plan of action to address the health concerns identified in East Hartford.
The CHNA was completed in close partnership with local stakeholders including health and social service providers, elected and appointed officials, community-based organizations, faith leaders, the public schools, and community residents. This report (attached below) provides an overview of data gathered from a stakeholder survey, rapid community assessment, and the community health survey, as well as the overall key findings.
