Request a Copy of LOST East Hartford Parks and Recreation Youth ID

To request a copy of a LOST Parks and Recreation Youth ID, please complete the information below.  Please note that the same fee applies for a lost ID reprint as a new ID ($6.00).  The expiration date for a lost ID reprint would be the original expiration date and would not be extended. Therefore, we recommend completing the request and uploading the required documentation for a NEW Parks and Recreation ID instead.

Waiver of Liabiltiy
I submit that the information stated above is truthful and accurate, and that the child for whom I am requesting a youth ID is a permanent resident at the address listed above. I understand that the East Hartford Parks and Recreation Department may contact the school listed to confirm the home address and that the child is currently enrolled. The East Hartford Parks and Recreation Department also reserves the right to request additional information confirming residency (mortgage/lease/rental agreement/utility bills) if necessary. I understand that East Hartford Aquatic Facility Rules require proof of residency for anyone entering the facilities, and I understand that children 11 and under must be accompanied by a responsible adult age 16 or older, at all times while in the facility. I agree to abide by all facility rules, and understand that pool use is a privilege that may be revoked at any time.
