Memorial Day Parade Volunteer Registration 2024

Consent – Assumption of Risk – General Release PERMISSION TO USE IMAGE OR LIKENESS: I consent to photos or videos being taken of me participating in the East Hartford Memorial Day Parade (the “Event”). RISK AWARENESS VERIFICATION I am aware that certain risks and dangers may exist for me in participating in the Event. I represent that no guarantees have been made to me by the Town of East Hartford, or any of its boards, commissions, or volunteers, as to my proficiencies, skills or physical abilities to engage in this Event without suffering possible injuries. I understand that participation in the Event will involve a variety of physical activities and that there is a risk involved, which each participant must assume (including injury, disability, or death). I also understand that participation in the Event is at my request. I have made my own judgment that I am capable of participating in this Event, and that I should participate in this Event. ASSUMPTION OF RISK I do hereby, on behalf of myself, and my heirs, assigns, successors and executors, fully assume all risks associated with participation in the Event and agree not to sue and hereby release, indemnify, defend and agree to hold harmless the Town of East Hartford, and its elected officials, agents, servants, employees, boards, commissions and volunteers from any and all harm, injury, damages, claims, suits, demands and liability arising in any manner from death or injury (bodily or otherwise) to me, or any other party occurring during, or as a result of, my participation in the Event. I, the undersigned, acknowledge that I have read and understand all aspects of this form.
