Online Counseling Referral Form

East Hartford Youth Services has launched an online counseling referral form that can be completed and submitted directly through our webpage! This form makes it easy for parents, school staff, community providers and anyone else to submit a referral for counseling services without the hassle of back and forth phone calls. Referrals go directly to Counseling Coordinator Sarah Hornberger and families can expect a follow up within 1-2 days after a referral is submitted.
