Obtaining a Certified Copy of a Death Certificate:

• To request a certified copy of a Death Certificate by mail, please complete the request form (see link below), and follow the mailing instructions on the form.
• If requesting in person, you will need to fill out a request form and present a photo ID.
• There is a $20 fee for each certified copy.
