You must bring the following to your appointment:
1. Social Security numbers for all household members.
2. Current utility bill in household member's name of majority status (light and gas).
3. Oil applicant should have name of oil company.
4. Proof of gross income for all household members (if paid weekly-4 pay stubs, biweekly-2 pay stubs), must be consecutive for the 4 weeks prior to application date.
5. Current rent receipt or mortgage statement, if heat included in rent (must provide landlord's name, address, and phone number).
6. Current COMPLETE bank statement/s (all pages).
